Each applicant for water service shall sign an application form provided by the city giving the date of application, location of the premises to be served, whether the premises have been served before, the date on which the customer desires the service to commence, the purpose for which the service is to be used, the address for sending bills, the customer’s residence address, the class and size of the service and such other information as the superintendent may reasonably require. Rules, regulations and dates established by the city shall be a part of the contract between the city and the customer. The filing of an application for the use of city water shall be considered as consent by the customer to be bound thereby. (Prior code § 3.015)
